My IPad 3 contract has just ended, How can i unluck my ipad if the IMEI number is not recognised when attempting to send in the NUC form.

i am unable to send in the NUC request form as it does not recognise my IMEI number from my ipad, and i can therefore not submit the form.

I ill try one of those methods, as i have been able to submit the form, but i got an emails saying my post code, DOB and payment method were not recognised.  i know these are correct.
