SS not showing up on My Vodafone

Hi - for a few months now my phone hasn't been working with my Sure Signal although other phones have been working fine. I now would like to try and work out why, but for some reason the Sure Signal isn't listed in My Vodaone. If I try to add it I get an error message, presumably because it's already registered.

How can I go about fixing  it ? Can it be de-registered somehow ?
