1) Does the issue happen in just one location? If so, how far do you have to travel to regain service?
travelled 5 miles and still 4G connection problems

NO

UNABLE TO CONNECT TO INTERNET

ONLY A GEW DAYS AGO BUT NOT SURE AS HAF ACCESS TO WIFI BEFORE THAT.
7) Is the issue permanent or intermittent? If intermittent, are there certain times of the day when it occurs?

@Kpreater As you've stated this issue doesn't happen when you try your SIM card in a different phone, this could be an issue with your device.
