91£ Additional charges on my 3G

I've been paying £40 a month successfully for the past six months for my 4G, this includes that pass with all the extras, so I've never gone over it once.
This month however, on the 14th, only 11 days into the contract for the month, all of my data was used up. Not only that but I had gone over a few gigs already, thus acruing a hilarious bill for next month, that being £131.
This is all seems rather absurd as I've not done anything different this month.
Between the 3rd and the 14th, I have easily watched 100 to 200 videos on youtube(all of which should be covered by my pass) however, looking at the "plan usage" I have seemingly only used 250mb of data, which is imposible.
What is going on?

Having same problem and arguing now with Vodafone, i can demonstrate that the data is not being allocated to the video pass

Oh man, it's good to hear that they are at fault, it's a bit unfortunate they didn't say anything.
I have cancelled my contract (still got charged though) and have moved to a different service provider.
