Numerous Vodafone employees unable to fix my account to allow upgrade

I've spent several hours on the phone this week trying to get my broadband renegotiated. 

After being told that my account had an open disconnection (from 18 months ago when I switched from FTTC to FTTP) and that was preventing it being fixed, my account seems to be completely broken,  No one from the accounts team can change my existing contract.   How can I raise a complaint and get it escalated fixed?
