Help, my company signed me up without telling me, failed to pay bills...

Help, a company I used to work for has put a dongle contract in my name without telling me, when I left they also stopped making payments and didn't cancel the contract, this is now affecting my credit rating.  How can I get this removed??

My credit report is showing that I owe £67, but I have no details of the contract as I wasn't aware there was a contract in my name, not sure where to even start with getting this resolved and it is preventing me from getting a mortgage application agreed.
