If you are Pay monthly go into your My Vodafone account and Manage Settingd, from there you can get the PUK code.
If you are on PAYG Live Chat or 191 will be able to help you.
Are you entering the PUK code every time or does it ask for the PUK when your try to enter a new pin for the phone itself?
