Hi, i have the same issue, i know this is an old thread but its all i could find.
Iphone 4, monthly contract, internet allowance etc. hitting 'personal hotspot' on the phone tells me to go to vodafone.co.uk/help/iphone
with no avail obviously.

Hi James / other voda guys
same issue - had HTC with hotspot, moved to iPhone 4S & now no hot spot...
can you fix it please?

Hi Tech team,
I am having the same problem on my IPhone 4. Have tried resetting (all types) and strarted iphone from scratch but to no avail.

Further to my last. The problem is a network issue not the phone. For many who have changed tariffs or gone sim only at the end of a "pay for your phone" contract, the APN which enables the Personal Hotspot is dropped off the contract. The solution is to call Vodafone and askethem to restore the APN.
