I have just changed providers and am unable to get my iPad unlocked, I have sent the form on the web site with all the details all I get back is a msg could not find IMEI number on the account , been in to Vodafone shop they are a waste of time, rang Vodafone CS every day since 13/04/17 and still nothing. I am now even less happier with Vodafone, I need my iPad for my job and unable to use it.

Totally appreciate how frustrated you must feel about this.
Usually if something goes wrong with the online form you fill out then Customer Services can complete the form on your behalf.

I suspect the reason for this is due to you having cancelled your account. The quickest way and to save all the messing around is to get the ipad unlocked as PAYG. This means using the ipad on the network as PAYG for 30 days after which time you will be able to complete the data services part of form as PAYG and get the device unlocked. Please see below.
