thank you for your help but that link just takes me to the help and support page,and not the online form which needs to be filled out,if I'm missing something on the help & support page pls help,I've tried all the links available and they all bring you back to the same page,very frustrating!!

Hi, there is a problem accessing the link from mobiles, it has been reported.

It can be accessed using a PC ok.
