Further to my discussion with your telephone operator today.
Please can you provide me with proof of usage from the first and last date before I barred my phone due to it being lost as I have been asked by my insurance company to provide this information.

Proof of useage can be obtained by looking out your monthly bills if you're on pay monthly contract or on your 'manage account' if you're on PAYG, either way you will need to log on to the website from your computer to veiw your account details and print out the required information.
