Need help unlocking iPhone 4S

I've given my old iPhone 4S to my sister only to find out it's locked. I've attempted to fill out the unlock form multiple times but without luck.

After talking to the live chat it seems that Vodafone don't have the IMEI details because I bought it via Carphone warehouse, and that I should speak to them instead but CW make it clear that they can't help.

Reading the forums here it looks like someone here can actually help. I can provide proof of purchase if needs be.

Hi. This has come up before and CPW should be able to help by ensuring they have sent the imei to Vodafone.

Another way to get it sorted is for your sister to get a Vodafone Payg SIM, top it up and use for 30 days on the network, making a few calls, sending texts or using a bit of data. That will register the imei against her name and she can the apply for the unlock.
