Can anyone from Vodaphone please PM me as I have exhausted all options attempting to get issues aorund requesting an unlock code resolved?

Which options have you used already and exhausted ?
In your thread title you mention proof of purchase which is obtained via How-do-I-get-a-proof-of-purchase-usage-or-barring.
In your post you mention unlocking which is obtained via Vodafone Unlocking Form which can be achieved as long as you meet the criteria set in Nuc Unlocking.
Customer service on 191 or Live Chat should also be able to help.

You mention proof of purchase. Does that mean it's an iPhone and you want to talk to Apple?
