Itemised usage should show you all the transactions on your account over the past 90 days, and therefore show both the top up and the date when you bought the BVB. It will also show with a cost of 0 all the texts, calls and data you have used. The duration of the calls is shown, although of course they are rounded up to complete minutes, but you should be able to see how (or if) you used the 150 minutes included in the bundle.
However you really do need to talk to Live Chat or CS on 191, to find out more about your account, as there's clearly a problem of some sort with your account, although it isn't clear from the details you give, exactly what the problem is!
