Can't complete checkout during plan upgrade

When I try to checkout my upgraded phone and plan, it keeps redirecting me back to "Review your order"? No error or form information is displayed... seems to be a bug?

Steps:
1. Filled out form
2. Ticked "I agee to the terms and conditions"
3. Hit "continue"
4. "Processing your order" page is displayed then redirects back to the checkout form

Tried two different browsers on two different computers, same thing (so not a cache or browser issue) .

When upgrading to a phone and plan, this is done from this link Upgrade to Evo . 

If you need help use the online chat facility for direct support from an online advisor.

Thanks, I went through the ordering process with an advisor on "chat" and it's been now been sorted.
