How should i layout a purchase order?

Hi there

I am trying to switch my account from personal to business. To do this, i have to create a purchase order to vodafone. How do i lay this out? what information needs to be on the purchase order? And what address do I use to address it to vodafone?

To complete the credit credit Vodafone are looking to receive details of the numbers to be transferred on company headed paper, the documents required will need to be submitted with the Transfer of Ownership.
