cant get NUC code form to work

I've been trying to get my NUC code for a old galaxy A71 i got from vodafone, filled the form in four or five times, click the get code button, the page just cycles for a for about 2 minutes then get 

This site can’t be reached

new-forms.vodafone.co.uk took too long to respond.

is the service down?

Another route could be to get a Highstreet Store that deals in unlocking phone's for a nominal fee.

thats the page ive been using, just tried again on my phone, did the same just cycled then timed out.

Completing and submitting the form multiple times can be the reason.  Clear all cookies and try again, also try incognito mode.

just tried that and on a mobile device same result cycled for about 2 minutes then timed out

It won't submit if any information is incorrect.  You would need a Vodafone number and have used it in the device for 30 days.

its my wifes old phone that was on vodafone from vodafone it was used on vodafone for 2years

If the phone was on contract in your  Wife's name, to unlock the phone against her account it needs to be completed in your Wife's name. 

If it was PAYG the SIM needs to have been used in the phone in the last 30 days.

It sounds like the information doesn't match and thats stopping the form from submitting.

All contracts in my name, tried using my wifes number also still wouldnt submit.

form doesnt ask for name just number and email
