How do I claim a refund of the cost of leaving my old broadband provider from Vodafone? I believe this was offered when I placed my order.

I think the best way to get clarification on that is to call the Vodafone Broadband Team.

Note there is an option for them to call you back if they are busy which they seem to be all the time at the moment, saves you being on hold and don't loose your place in the queue.
