SIM swap service does not work ...

I am upgrading from an iPhone 4S to an iPhone 5S and I need a SIM for the new handset. The iPhone 4S is no longer working, so I cannot use the text method to activate a nano sim, and the SIM swap form (that I have now filled in twice correctly) keeps resulting in an email four hours later stating that something is wrong. My nearest Vodafone shop is a 100 mile round trip. All I want to do us to use my new IPhone 5S with the same number that I have had for years. How do I do this?
