I'm having a problem with my Note 4. My ringer volume is at its loudest but when anyone rings me, it does not ring out...what am I doing wrong....it had previously worked ok and I dont know what I've changed to alter it

Have you recently performed any software updates ?
i assume using the speakers and mica for calls , watching YouTube on loud speaker the volume is ok. Have you tried changing to a new ringtone.
One option is a Hard Reset which really is a last resort but if your unable to find anything in the phones settings it maybe an option.
Before carrying out this process which takes the phone back to how it came out of the box which deletes data saved in the phones memory its advised to perform a back up.
Option 1
Go into Setting > Backup and reset > Factory data reset. This will delete all your personal data so make sure to make a backup.
Option 2
Turn your phone off or make sure it’s turned off.
Press and hold the Volume Up, Home and Power buttons simultaneously.
When the phone vibrates, release the Home and Power buttons, but keep pressing Volume Up button.
This will launch the Android Recovery System. You can release all buttons when this happens.
From here, you can use the Volume buttons to navigate and the Power button to make a selection.
Select “wipe data / factory reset”.
Select “Yes — delete all user data”.
The process will take place. When done, just select “Reboot system now” to restart your device.

Hello.
Can you please first check that the ports and speakers are clear of dust and lint.
Have you fitted a new case or screen shield recently.
Try a soft reboot by turning it off , remove the battery (handset applicable if battery removes) , reinsert battery and turn back on.
Ensure re you haven't put the phone in Mute Mode.

Perhaps try choosing a music track as a ringtone to see if that works.
