I have a Doro 5030 locked to Vodafone. Doro is not listed as a manufacturer on the online Network Unlock request form. So how do I complete the forn/apply for an unlock code?

Usually filling out the Unlocking Form is enough as long as all the criteria is met.
If not then customer services on 191 or Live Chat can fill out the form on your behalf.
