How do claim your free Amazon Fire Tablet?

After 5 separate conversations with customer services, I FINALLY have a text with the code to claim my free Amazon Fire Tablet. Unfortunately, I have zero instructions on how to use the code to claim it. Amazon doesn't recognise the code. Can anyone help? I can't face contacting customer service yet again!

I assume you add it on the checkout page. Make sure you are ordering the All-new Fire HD 8 tablet | 8-inch HD display, 32 GB. 

 If it still doesn't work, I can only assume the offer has expired.
