It started off with no MMS, then progressed to no Internet at all when outside WiFi range. Repeated phone calls to Vodafone and all I got was 'we'll send you some settings by text, should work after midnight'. This happened about 4 or 5 times before someone tried to talk me through some different settings. Again, no joy. Eventually, after about 10 calls to the 191 line, I was directed to '2nd line support' who inform me that the Galaxy S4 needs an update and it's all Samsung's fault. Nothing they can do. Don't know why they couldn't have said this in the first place, meantime I am paying for MMS and Internet and not getting the service. They have offered me £5 off my bill, but still no word on when I can have a full and proper MMS / Internet service that I am paying for. A trawl of various message rooms reveals that this goes back to around October last year. Are Vodafone still selling S4s knowing that they don't work? Is there a fix that they don't know or haven't told me about? When can we expect full service to resume? If they don't know, can I exchange the (contract) S4 for a phone that WILL work properly?

If you've successfully installed the Settings (APNs), that side of things should be OK, though, if the phone was working OK before, there's no particular reason why it should suddenly forget its identity.
Assuming for a moment that this isn't an account issue (which is pretty unlikely), any problem is likely to be with the device itself. In the first instance, I think we're looking at a factory reset. If you've recently updated it, this is desirable anyway as it clears out any bits of old code that are hanging around and confusing it. A recent update does seem to have necessitated this more than usual on Samsung phones.
Don't forget to back up your content first and, if you're using an external sd card, remove it as the process sometimes deletes files there too. If the phone went off for repair, this would be part of the process, so you won't have lost anything by doing it yourself. When you restart, choose the Restore option when it's offered and your apps will re-load by themselves.

I have been having exactly the same issue since 14/1 - no internet (except wifi) - Vodafone keep sending me texts saying we will send youa text with the new settings shortly but they have been saying that for 5 days now! I am getting really cross and frustrated and as usualy Vodafone don't seem to give a monkeys...

APNs tend to install silently in background on recent systems, which may be why nothing appears to have happened. You can also go into Settings|Networks|Mobile Data|More and see if the "get settings from provider" option is there. If it is, tap it and it'll install the correct APNs and delete and others. If not, the phone thinks everything is OK.
It could also be (especially if you've recently done a system update) that the phone needs a factory reset. Don't forget to back up your content first. Samsung devices seem to be particularly sensitive to this and it clears out any bits of the old code that are cached and confusing it.
